We often expect people who lead to have an answer ready. That expectation can lead them to gloss over the dilemma just when talking to the team would help lead to a better decision.
Admitting that we do not know something does not mean leaving the responsibility to others. We can clearly say what we are missing, who we will consult and when we will decide.
The same goes for a mistake. Explaining what will be fixed is more useful to the team than defending the impression that everything was under control. The leader remains responsible, but they do not have to feign infallibility to carry that responsibility.
