A manager can bring useful information and express it in a way that makes it difficult for the team to respond. An abrupt interruption, impatience, or a raised voice shifts attention away from the work and towards the manager's reaction. People begin choosing their words according to what will cause the least upset.

At that point, saying the door is always open tells us little about the conversation that actually takes place. The measure is what happens when someone asks an uncomfortable question or points out a mistake. After that moment, the team knows better how much room it has for an honest response.

Dissatisfaction with a result requires clarity: what was not done, what the consequences are, and what we are changing. Humiliating a person obscures all three questions. The manager's tone becomes part of the working conditions, and responsibility for it remains theirs even when the problem is serious.
