I believe that some business difficulties arise when we deal with form more than purpose. Reports, procedures and controls can be useful, but their number does not show how well we understand people and the actual business.
It is important to check which decision a certain report enables and which problem the procedure prevents. If there is no clear answer, perhaps we are just shifting time from doing the work to describing it.
Leadership requires being present and talking about what makes it difficult for people to do what is agreed. This does not eliminate the need for organization. It gives it meaning, because the rules can be adapted to real circumstances and responsibilities that should be clearly divided.
